Chest pains, hard to breath, hard to sing, and cough along with dry throat?

I have chest pains, and its hard to breath, hard to sing, and I have a cough along with dry throat. The chest pains are right in the middle of my chest. These symptoms come and go every day. I am only 13 years old!! I have had some dizziness, but that's probably because I'm not drinking enough water. And the cough is really bad. What is this?!
